The Patient-Partner Program (PPP) is an important new program that KFCP hopes to replicate throughout our service area. A curriculum has been developed to train and certify ESRD patients, and family members, who are medically stable and who have adjusted successfully to their own disease, or to that of their family member, to serve as mentors (Patient Partners) for newly diagnosed ESRD patients and other ESRD patients seeking emotional support. Once certified, the Patient Partners will be available for one-on-one visits with patients and their families either in the hospital,in the patient's home, or at a place of the patient's choosing.

The support offered by the Patient Partner is designed to complement the care given by medical and allied health professionals.
